Worker crushed under grinder

BRAMPTON — A $110,000 fine has been levied against a Toronto company in connection with the death of a worker.

Furfari Paving Co. Ltd. was issued the penalty in mid-July for its failure under Ontario’s Occupational Health and Safety Act to ensure the operator of a vehicle was assisted by a signaller when the path of travel was obstructed.

On August 22, 2008, two employees were operating an asphalt road milling machine, known as a grinder, to remove the top layer of asphalt from a section of road in Mississauga, Ontario. Other machines were in use nearby, including a pavement cutter operated by a third worker, who was cutting and sealing cracks after the grinder had passed through, notes a statement from Ontario’s Ministry of Labour (MOL) in Toronto.

When the two workers finished removing the layer of asphalt, they put the grinder in reverse. When they stopped, the MOL notes, they saw that the third worker had been caught underneath their machine and crushed.
