Perhaps charges under OH&S Act were not appropriate to this, but I am guessing there may have been a civil suit with good litigators.

The out-of-court settlement between the family and the city was worth mentioning but, I was really hoping to glean some clues as to why the city was not found guilty.Or at least, direction on where that information could be found.Thankswk
